Youth Basketball Training
Join our Friday night practices at St-Henri High School Gymnasium. Each session is designed to help kids build skills, teamwork, and confidence in a fun and structured environment.

Day & Time: Fridays at 7:00 PM
Duration: 2-hour practice (7:30–9:30 PM)
Location: St-Henri High School, 4115 St-Jacques St, Montreal, QC

Age Groups:

U10 (6–10 years old) – Skill fundamentals, fun games, teamwork basics
U13 (11–13 years old) – Intermediate drills, basketball IQ, team play
U16 (14–16 years old) – Advanced skills, game strategies, competitive development
U21 (17–21 years old) – High-level development, conditioning, competitive gameplay

Season:

Fall 2025 – September 26, 2025 to December 19, 2025
Winter 2026 – January 9, 2026 to May 1, 2026
Summer 2026 – TBD

1. Participation Acknowledgment
I, the undersigned parent/guardian, give permission for my child to participate in the Montreal Mynix Athletic Club Youth Basketball Program. I understand that basketball involves physical activity and carries a risk of injury.

2. Medical Consent
• I confirm that my child is physically fit to participate.
• I agree to inform the program organizers of any medical conditions, allergies, or medications.
• In case of emergency, I authorize staff/volunteers to administer first aid and/or secure proper medical treatment.

3. Liability Waiver
I release Montreal Mynix Athletic Club, its coaches, volunteers, and staff from any liability for injuries, accidents, or losses sustained during practices, games, or related activities, except in cases of proven negligence.

4. Photo & Video Release
I understand that photos and videos may be taken during practices, games, and events.
By completing this form and checking this agreement, I give permission for my child’s image/video to be used for promotional, educational, and social media purposes.

5. Code of Conduct
• My child will respect coaches, referees, teammates, and opponents.
• Unsportsmanlike behavior may result in suspension or removal from the program.

6. Transportation & Supervision
I acknowledge that it is my responsibility to arrange safe transportation for my child to and from practices/games. Organizers are not responsible for children left unsupervised outside scheduled program hours.
